Key Responsibilities:
  • Conduct comprehensive assessments to identify patients' needs and develop personalized care plans
  • Manage patients through acute medical situations, chronic conditions, and rehabilitation episodes
  • Ensure accurate and complete documentation, adhering to ICD-10 coding standards and CMS regulations
  • Collaborate with interdisciplinary care teams to facilitate seamless care transitions and coordination
  • Provide patients and caregivers with education, counseling, and support to promote optimal health outcomes

Requirements:
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (for Nurse Practitioners) or accredited Physician Assistant degree program (for Physician Assistants)
  • Active and unrestricted licensure in the state of practice or ability to obtain by start date
  • Board certification through the American Academy of Nurse Practitioners or American Nurses Credentialing Center (for Nurse Practitioners) or National Commission on Certification of Physician Assistants (for Physician Assistants)
  • Ability to work independently with minimal supervision and navigate various dwelling conditions
Preferred Qualifications:
  • 1+ years of clinical experience in senior living, geriatric medicine, or home care
  • Experience in meeting the healthcare needs of patients with complex behavioral, social, and functional needs
  • Understanding of geriatrics and chronic illness, as well as advanced illness and end-of-life discussions
