About the Role

We are seeking a highly skilled Advanced Practice Provider to join our team at Minnesota Oncology. As a Palliative Care Advanced Practice Provider Float, you will play a critical role in providing exceptional care to our patients.

Key Responsibilities
  • Assess, plan, implement, and evaluate care for patients with complex needs.
  • Initiate and document appropriate interventions, evaluate patient/family response, and prescribe therapies as indicated.
  • Perform procedures and mentor/train other providers as needed.
  • Assess patient/family needs and provide education on disease management, treatment options, and wellness strategies.
  • Promote wellness and recommend community resources to meet patient and family needs.
  • Consistently incorporate evidence-based research into clinical practice.
  • Complete patient charge capture and medical record documentation in a timely fashion.
  • Provide case management for patients and their families, and serve as a resource for nursing staff.
  • Provide palliative care for patients and their families as needed.
  • As a subject matter expert, promote and provide staff education on palliative care and related topics.
Requirements
  • Nurse Practitioner or Physician Assistant with a Master's degree from an accredited program.
  • Current licensure and certification as a Nurse Practitioner or Physician Assistant in the state of Minnesota.
  • Minimum of three years of experience as a practicing NP or PA, with a strong preference for experience in oncology.
  • Excellent analytical, leadership, and organizational skills, with the ability to multi-task and prioritize tasks effectively.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
