Advanced Practice Provider in Palliative Care

CentraCare Health System is seeking an Advanced Practice Provider in Palliative Care
This role involves collaborating with a dedicated multi-disciplinary team, including Advanced Practice Providers, Physicians, Registered Nurses, a Chaplain, and a Psychotherapist.
The Palliative Care team is committed to alleviating the symptoms, pain, and stress associated with serious illnesses for both adults and children. The primary aim is to enhance the quality of life for patients and their families. As part of this team, you will provide essential support throughout the patient's journey.
Key Responsibilities:
The main responsibilities of the Palliative Care Advanced Practice Provider include:
- Consulting on care goals, which may involve facilitating family discussions at the healthcare facility.
- Managing symptoms effectively.
- Addressing end-of-life care requirements.
- Providing training, education, and fostering relationships to support palliative care initiatives.
Team Collaboration:
The Advanced Practice Provider will actively participate as a member of the palliative care team, offering consultations to:
- Medical personnel.
- Patients and their families.
- Nursing staff.
- Social services.
- Spiritual care professionals.
Work Schedule:
- Standard shifts are Monday to Friday, from 8:00 a.m. to 4:30 p.m.
- This position is 0.75 FTE and is eligible for full-time benefits.
- No weekend shifts are required (subject to future changes).
- No holiday shifts are required (subject to future changes).
- No on-call duties are required (subject to future changes).
Educational Requirements:
A graduate degree from an accredited nursing program is required. Candidates must possess a Master's degree with formal training as a nurse practitioner or clinical nurse specialist in the relevant specialty area. A minimum of 500 supervised clinical hours in palliative care or a related field is preferred.
Experience:
Preferred candidates will have experience in Hospice or Palliative Medicine, with additional preference for those with geriatric and oncology backgrounds. ACHPN (Advanced Certified Hospice and Palliative Nurse) certification is required within two years of employment. Strong communication skills, leadership abilities, and clinical expertise in managing patients with life-limiting conditions are essential.
Licensure Requirements:
Current licensure as a registered nurse and as an Advanced Practice Registered Nurse in the State of Minnesota is mandatory. Certification as a nurse practitioner or clinical nurse specialist in Minnesota is also required. ANCC Board Certification as a Clinical Nurse Specialist is necessary.
Applicants must have a written prescribing agreement with a physician, adhering to standards set by relevant nursing and medical associations.
A current American Heart Association (AHA) BLS Healthcare Provider card is required prior to completing orientation.
Successful completion of medical staff office credentialing and third-party payor credentialing is necessary before working independently and billing for services.
