Overview:


Fairview Health Services is in search of an Advanced Practice Provider to collaborate with designated operational leaders in delivering guidance for the Care Map program services throughout the organization.

The Program Medical Director role encompasses 0.2 FTE and up to 0.8 FTE in a clinical setting.

Job Responsibilities:
Collaborates with the Care Map Program Manager, CVR System Director, Care Map Medical Directors, and operational leaders to establish the strategic vision and objectives of the Care Map Program.

Formulates and executes clinical protocols/tools aligned with best-known practices alongside the CQVC team.

Facilitates communication, education, and ensures adherence to the application of these protocols/tools.

Partners with fellow Medical Directors and the broader leadership team to bolster the strategic goals and transformation initiatives of both the division and the organization.

Actively seeks opportunities to enhance knowledge of best practices and emerging developments within related fields, integrating this knowledge into program operations.

Comprehends evidence and collaborates with teams to develop evidence-based protocols.

Employs data to inform decisions and prioritize care map program initiatives.

Understands and collaborates with clinical operations on clinical workflows to support care maps.

Promotes care map engagement with clinical teams, as well as medical, clinical, and operational leaders.

Qualifications:
Required

Graduation from an accredited Physician Assistant program.

Board Certified/Board Eligible through NCCPA.

Possession or eligibility for a PA license with prescriptive authority through the Minnesota Board of Medical Practice.

Meets threshold criteria/qualifications for Credentialing and Privileges.

A minimum of 3 years of clinical experience.

Preferred:
3 years of experience in a leadership capacity or with experience in leading and executing systemwide projects.

Experience in quality enhancement.

Familiarity with evidence standards and/or protocols.

Experience in data-driven decision-making.
