Advanced Practice Provider in Palliative Care

About the Role:
The Palliative Care team at CentraCare Health System is seeking an Advanced Practice Provider, which may include a Nurse Practitioner, Physician Assistant, or Clinical Nurse Specialist.
This role involves collaborating with a diverse team consisting of Advanced Practice Providers, Physicians, Registered Nurses, Chaplains, and Psychotherapists.
The primary focus of our Palliative Care team is to offer relief from the symptoms, pain, and stress associated with serious illnesses for both adults and children. The aim is to enhance the quality of life for patients and their families. As highlighted by our Medical Director, the journey of Palliative Care can be likened to a road trip, where you will provide essential support throughout.
Key Responsibilities:
The main responsibilities of the Palliative Care Advanced Practice Provider include:
- Consultation regarding care objectives, which may involve facilitating family discussions for patients.
- Management of symptoms to ensure patient comfort.
- Addressing end-of-life care requirements.
- Providing training and education while fostering internal and external relationships to support palliative care initiatives.
Team Collaboration:
The Advanced Practice Provider will work closely with the palliative care team to deliver consultations to:
- Medical professionals.
- Patients and their families.
- Nursing staff.
- Social services personnel.
- Spiritual care providers.
Work Schedule:
- Standard shifts are Monday to Friday, from 8:00 a.m. to 4:30 p.m.
- This position is 0.75 FTE and is eligible for full-time benefits.
- No weekend shifts are currently required.
- Holidays and on-call duties are not expected at this time.
Qualifications:
Education: A graduate degree from an accredited nursing program is mandatory. A Master's degree with formal training as a Nurse Practitioner or Clinical Nurse Specialist in the relevant specialty is required, along with a minimum of 500 supervised clinical hours in palliative care or a related field.
Experience: Preferred candidates will have experience in Hospice or Palliative Medicine, with additional preference for backgrounds in geriatrics and oncology. ACHPN certification is required within two years of employment. Strong communication and interpersonal skills, along with leadership capabilities and clinical expertise in managing patients with life-limiting conditions, are essential.
Licensure Requirements:
Current licensure as a Registered Nurse and Advanced Practice Registered Nurse in the State of Minnesota is required. Certification as a Nurse Practitioner or Clinical Nurse Specialist in Minnesota is also necessary. ANCC Board Certification as a Clinical Nurse Specialist is mandatory.
A written prescribing agreement with a physician, in accordance with the standards set by the Minnesota Nurses Association and the Minnesota Medical Association, is required.
Additionally, candidates must possess a current American Heart Association (AHA) BLS Healthcare Provider card prior to the completion of orientation.
Successful completion of medical staff office credentialing and third-party payor credentialing is required before independent practice.
