As a Palliative Care Navigator (MSW or LPN) at Agape Care, you’ll provide patients with individualized assistance across the care continuum to patients, families, and caregivers to expedite and coordinate care, and address health system barriers. The Palliative Care Navigator assumes responsibility for the coordination of patient care through interdisciplinary and multidisciplinary collaboration to achieve optimal patient outcomes with a focus on high-risk patients or those with complex care needs (including multidisciplinary care). The Palliative Care Navigator has a particular focus on improving transitions in care. Additionally, the Palliative Care Navigator functions as a clinical advocate and educator for patients and families. The Palliative Care Navigator assists to build and maintain community relationships to provide expedient and reliable access to quality care. The Palliative Care Navigator identifies and alleviates stressors during transitions and barriers to care with the aim to deliver a seamless model of care that benefits patients, family members, providers, and the healthcare team.

Qualifications:
  • A heart to serve patients and families and a passion for providing the best possible care
  • Education: Current state license as a LPN or MSW. (SOCIAL WORKER PREFERRED)
  • Experience: Minimum of 5 year experience in a health care setting required. Three years of Hospice and/or Palliative Care required
  • Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs.
