The Advanced Practice Nurse is a licensed professional nurse with advanced education and clinical expertise who delivers comprehensive hospice care in accordance with state regulations and under the guidance of a collaborating physician.

This role involves providing hospice services through thorough evaluation and assessment of patient health histories, promoting health, diagnosing medical conditions, and managing health issues by formulating and implementing care plans, prescribing medications or treatments, and collaborating with patients, families, and other healthcare professionals.

Under the direction of a physician, the advanced practice nurse conducts patient visits, performs physical assessments, and evaluates hospice eligibility in collaboration with the hospice interdisciplinary team.

The Advanced Practice Nurse utilizes community resources to design and direct services that address the needs of individuals and families within their homes and communities.


Key Responsibilities

Patient Care:

Delivers medical care, alleviates symptoms, and manages terminal illnesses in compliance with state regulations under the supervision of a collaborating physician.

Conducts comprehensive history and physical examinations by assessing the patient's medical condition and health history, ordering and interpreting laboratory and diagnostic tests, diagnosing health conditions, and documenting findings in the patient's medical record.

Manages health conditions and terminal illnesses by creating individualized care plans, prescribing medications and treatments, obtaining consultations, and coordinating hospice services.

Completes required face-to-face visits to assess hospice eligibility based on established criteria and local coverage determinations.

Provides clinical information to the hospice medical director for eligibility interpretation and re-certification.

Recommends patient eligibility for hospice care certification and re-certification to the hospice medical director.

Reviews clinical information and collaborates with the hospice interdisciplinary team and other healthcare professionals to oversee patient care.

Ensures patients receive appropriate symptom management through collaboration with team members.

Develops treatment protocols and proposes intervention options based on current literature in collaboration with the supervising physician.


Communication:

Engages with the interdisciplinary team to create, review, and update the patient's care plan.

Consults with the patient's attending physician, hospice medical director, and other healthcare professionals regarding ongoing care needs and medical management related to the patient's terminal condition.

Coordinates care with other community health practitioners.

Participates in interdisciplinary team meetings.

Maintains accurate clinical documentation, including physical examination notes, assessment visit notes, medical orders, and collaboration notes.

Educates and counsels patients, families, and caregivers on preventative care, medical issues, psychological concerns, and spiritual needs in conjunction with the interdisciplinary team.

Ensures a safe environment for patients and assists families in providing continuity of care.

Collaborates with families and the hospice interdisciplinary team to identify care goals and meet the needs of patients and families.

Serves as a nursing resource for consultation and education to team members and other healthcare professionals in the community.

Builds and maintains relationships with staff and community referral sources to support program growth.


Additional Responsibilities:

Stays informed of and complies with current Medicare/Medicaid, state, and federal regulations governing hospice services.

Ensures adherence to Medicare conditions of participation and other state regulations related to healthcare provision.

Complies with all HIPAA requirements in accordance with federal, state, and organizational policies.

Participates in quality monitoring and improvement initiatives for medical services.

Assumes responsibility for personal and professional development through continuing education and training.

Demonstrates knowledge in communication and counseling patients and families regarding end-of-life issues.

Qualifications:

Graduation from an accredited nursing school and nurse practitioner program.

Current nursing licensure and CPR certification.

Master's degree with at least one year of nurse practitioner experience and one year of hospice or palliative care experience.

Accreditation from an approved certifying body for advanced practice nursing as required by state regulations.

Certification in a specialty area preferred (e.g., Hospice and Palliative Nursing, Pediatrics, or Geriatrics).

Strong observation, communication, problem-solving, and nursing skills.

Ability to perform physical tasks, including lifting and transferring patients.

Must possess a valid driver's license and have access to a reliable vehicle that meets state and organizational insurance requirements.
