We are seeking a highly motivated and compassionate Clinical Case Manager to join our Intensive Transition Team. In this role, you will work closely with clients, families, and healthcare providers to develop and implement personalized care plans that promote recovery, stability, and overall well-being.

Job Description

As a Clinical Case Manager, you will be responsible for providing case management services to individuals with behavioral health issues, including assessment, goal-setting, and plan implementation. You will work collaboratively with multidisciplinary teams to coordinate care and ensure seamless transitions between services.

You will also develop and maintain strong relationships with community agencies, healthcare providers, and other stakeholders to promote access to resources and services. Utilizing evidence-based practices and trauma-informed approaches, you will engage clients and promote positive outcomes.

Responsibilities:
  • Provide direct care and case management services to clients, including assessment, goal-setting, and plan implementation.
  • Collaborate with multidisciplinary teams to coordinate care and ensure seamless transitions between services.
  • Develop and maintain strong relationships with community agencies, healthcare providers, and other stakeholders to promote access to resources and services.
  • Utilize evidence-based practices and trauma-informed approaches to engage clients and promote positive outcomes.
Requirements:
  • Bachelor's degree in psychology, counseling, or a related field.
  • At least one year of experience working with individuals with behavioral health issues.
  • Strong communication and interpersonal skills, with ability to build rapport with diverse populations.
  • Knowledge of community resources and services, with ability to navigate complex systems.
  • Ability to work independently and as part of a team, with strong problem-solving and adaptability skills.

**Estimated Salary:** $55,000 - $65,000 per year, based on experience and qualifications.
