About the Role
This position involves collaborating with multidisciplinary teams to identify patient needs and develop effective transition plans. You will work closely with patients, families, and healthcare providers to ensure continuity of care and successful transitions.
Your responsibilities will include:
- Assessing patient needs during discharge planning
- Providing resources and community referrals for continuity of care
- Care coordination with families, providers, and clinical teams
- FOLLOWING MEDICARE CONDITIONS WHEN NEEDED
- Utilization Reviews
Requirements
To be successful in this role, you will need:
- A Bachelor's degree in a healthcare-related field or equivalent experience
- A current BSN or commitment to achieving the degree in a specified amount of time
- 3 - 5 years of case management-related health care experience
- CCM or ACM preferred
- Excellent clinical documentation and successful usage of EMR systems

SolutionHealth offers a competitive salary range of $75,000 - $90,000 annually, depending on experience. We also provide a comprehensive benefits package, including health, dental, prescription, and vision coverage for full-time & part-time employees.
