Position Overview
As a member of the Prevea Clinic organization, you will be expected to deliver outstanding care to our patients while valuing the contributions of your colleagues. The Advanced Practice Provider (APP) is an advanced practice nurse or physician assistant equipped to deliver comprehensive health care services to individuals, families, and communities, with a focus on health promotion and disease prevention. Responsibilities include collaboration and coordination with physicians and other healthcare professionals in clinical practice, education, consultation, research, and program development.
Key Responsibilities
- Conduct thorough health assessments for patients with acute and chronic conditions.
- Evaluate and manage care for acute, minor, and episodic illnesses and injuries.
- Provide guidance to patients and families on health and illness prevention.
- Perform follow-up examinations.
- Assess, evaluate, and manage clients with both acute and chronic health issues.
- Work collaboratively with patients to identify their physiological, emotional, and environmental needs and priorities.
- Offer case management services to assist clients and families in accessing necessary services.
- Address patient health concerns through telephone triage and consultations as required.
- Provide services that may include ordering diagnostic and laboratory tests, prescribing medications, and recommending non-pharmacological treatment options.
- Maintain and review patient records, charts, and other relevant information.
- Conduct patient callbacks, report diagnostic findings, and provide consultations.
- Engage with physicians in reviewing patient health records.
- Participate in call coverage as needed.
- Attend mandatory department meetings and contribute to committees as requested.
- Perform other clinical duties as assigned by supervising physicians, in line with experience.
- Encourage ongoing quality improvement initiatives.
- Document all patient interactions in the electronic medical record.
- Graduation from a master's level nurse practitioner program or an accredited physician assistant program is required.
- Experience requirements may vary.
- Openness to learning new roles within the scope of practice.
- Ability to foster and maintain effective working relationships.
- Strong organizational skills to ensure timely patient appointments and appropriate follow-ups.
- Exhibit a courteous and service-oriented demeanor through positive interactions with patients, families, physicians, and colleagues.
- Engage in APP activities throughout Prevea and the community.
- Participate in community initiatives.
- Must possess the necessary licenses/certifications as a Nurse Practitioner or Physician Assistant.
- Nurse Practitioner: Registered nurse in the state of Wisconsin with certification from the American Nurses Credentialing Center (ANCC) or American Nurse Association (ANA). Must also be certified in Wisconsin as an Advanced Practice Nurse Prescriber or be eligible for certification, obtaining it at the earliest opportunity upon hire.
- Physician Assistant: Licensed by the state of Wisconsin as a Physician Assistant. Certification from the National Commission on Certification of Physician Assistants is required upon hire.
- CPR/Advanced Cardiac Life Support certification for Healthcare Providers from an approved program is preferred upon hire.
- Ability to hear whispers at distances less than 3 feet.
- Ability to hear whispers at distances between 3-8 feet.
- Color discrimination capabilities.
- Near vision correctable to Jaeger 2 or 20/30 binocular.
- Distance vision correctable to Snellen chart 20/30 binocular.
