About the Role:
We are seeking a highly skilled and experienced Clinical Nurse Specialist to join our team at Advocate Health. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to the development of our clinical services.
Key Responsibilities:
- Assessment and Care Planning: Conduct thorough assessments of patients to identify their healthcare needs and develop individualized care plans to address these needs.
- Medication Management: Administer medications, including immunizations, antibiotics, and other medications as prescribed by physicians.
- Patient Education: Provide patients and their families with education and support to promote healthy behaviors and self-care.
- Collaboration and Communication: Work closely with physicians, nurses, and other healthcare professionals to ensure seamless care coordination and effective communication.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient outcomes and improve clinical practices.
Requirements:
- Education: Associate's Degree in Nursing.
- Experience: Prior triage experience preferred. Recent Epic experience required.
- Licensure and Certification: Registered Nurse license issued by the state in which the team member practices, and Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required.
Physical Requirements and Working Conditions:
- Physical Demands: Must sit, stand, walk, lift, squat, bend, twist, crawl, kneel, climb, and reach above shoulders throughout the workday.
- Visual and Auditory Requirements: Must have functional speech, vision, hearing, and touch with ability to use fine-hand manipulation skills.
