Position: Social Work Assistant

Work Schedule: Four 10-hour shifts - Thursday, Friday, Saturday, Sunday - 8:00 AM to 6:30 PM

KEY RESPONSIBILITIES:

As a Social Work Assistant, your primary role is to aid case managers and social workers in supporting patients and their families in identifying and utilizing both internal and external resources to address social, emotional, and environmental challenges. This includes tasks such as verifying benefits, securing insurance authorizations, coordinating referrals, and accurately inputting data into clinical information systems.

MAIN DUTIES:
  • Provide administrative support to the department.
  • Generate standardized reports as requested, ensuring timely and accurate delivery.
  • Maintain precise and current documentation within departmental records.
  • Exhibit strong organizational skills and a sense of urgency in task management.
  • Establish, organize, and maintain departmental files and forms.
  • Schedule meetings effectively, considering subject matter, urgency, and required time.
  • Arrange details for patient and family conferences for department members.
  • Conduct audit activities under the guidance of Case Management Managers.
  • Engage with case managers and social workers daily to discuss patient discharge planning needs.
  • Contact referral agencies to facilitate post-discharge arrangements for patients.
  • Assist in scheduling follow-up appointments as requested by case managers.
  • Notify case managers, social workers, and patients/families of scheduled appointments, documenting them in the electronic health record.
  • Support case managers or social workers by providing options for post-acute care providers.
  • Inform designated facilities of admission needs and verify bed availability.
  • Update facilities regarding patient discharge conditions and final plans.
  • Prepare and send necessary documentation to accepting agencies to ensure continuity of care.
  • Coordinate transportation as directed by the Case Manager.
  • Demonstrate knowledge of local discharge planning resources.
  • Document discharge planning activities in accordance with departmental policies.
  • Communicate regularly with case managers and social workers about the discharge planning status of all patients.
  • Collaborate with the interdisciplinary care team to discuss patient discharge plans with families or representatives.
  • Work with case managers and social workers to confirm discharge dates and verify transportation arrangements.
  • Ensure that authorizations for post-discharge services are obtained when necessary.
  • Update case managers and social workers on the progress of assigned discharge planning tasks throughout the day.
  • Adhere to safety protocols and report any unsafe conditions or violations promptly.
  • Perform additional related duties as required.
EDUCATIONAL AND EXPERIENCE REQUIREMENTS:
  • High School diploma or equivalent is required.
  • A Bachelor’s degree in social work or a related field is preferred.
  • Three years of experience in a support role within care management may substitute for an associate degree.
  • Experience in home health care or managed care is preferred.
  • An equivalent combination of education and experience may be considered.
SPECIAL SKILLS:

The ability to manage multiple tasks effectively in a fast-paced environment is essential.
