About the Role

We are seeking a highly skilled and compassionate Social Work Specialist to join our team at ChristianaCare. As a Master of Social Work, you will play a critical role in providing exceptional care to our patients and their families.

Key Responsibilities
  • Collaborate with our Care Management Triad Team to resolve psycho-social barriers and support discharge needs.
  • Work closely with RN Case Managers and Utilization Management to ensure seamless patient care and progression.
  • Develop and implement effective care plans to meet the unique needs of our patients.
  • Provide emotional support and guidance to patients and their families during times of crisis.
Requirements
  • Master's degree in Social Work (MSW) or Master of Social Services degree (MSS) from an accredited graduate school.
  • Licensed CSW or MSW as required by the state of Delaware.
  • Acute Care Social Work experience preferred.
  • Maternal-Child position requires experience working with neonates, children, and families.
