Job Summary

We are seeking a skilled and compassionate Hospice Clinical Social Worker to join our team. As a Hospice Clinical Social Worker, you will play a vital role in providing emotional, social, and spiritual support to our patients and their families, helping them navigate the complexities of end-of-life care.

Key Responsibilities
  • Conduct comprehensive assessments of patients' social, emotional, and spiritual needs
  • Develop and implement individualized care plans to address patients' needs and goals
  • Provide counseling and support to patients and their families, addressing issues related to grief, loss, and bereavement
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
  • Document patient progress and outcomes in accordance with regulatory requirements
Requirements
  • Master's degree in Social Work (MSW) or related field
  • Valid license to practice as a clinical social worker in the state of agency operation
  • Experience working in hospice or palliative care settings
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
