Position: Social Work Assistant

PRIMARY ROLE:
The Social Work Assistant plays a vital role in aiding case managers and social workers in facilitating support for patients and their families. This includes helping them identify and access both internal and external resources to address their social, emotional, and environmental challenges. Responsibilities include verifying benefits, securing insurance authorizations, coordinating referrals, and accurately entering data into clinical information systems.
KEY RESPONSIBILITIES:
- Provide administrative support to the department.
- Generate standardized reports as needed, ensuring timely and accurate delivery.
- Maintain precise and current documentation within departmental records.
- Exhibit strong organizational skills and a sense of urgency in prioritizing tasks.
- Organize and maintain departmental files and forms effectively.
- Schedule meetings thoughtfully, considering subject matter, urgency, and time requirements.
- Arrange details for patient and family conferences, collaborating with department members.
- Conduct audit activities under the guidance of Case Management Managers.
- Engage with case managers and social workers daily to review discharge planning needs and the status of ongoing activities.
- Contact referral agencies to coordinate post-discharge arrangements for patients.
- Assist in scheduling follow-up appointments as directed by case managers.
- Notify case managers, social workers, and patients/families of arranged appointments, documenting them in the clinical system.
- Support case managers or social workers by providing options for post-acute care providers.
- Communicate with designated facilities regarding admission needs and verify bed availability.
- Update facilities on patient discharge conditions and final plans.
- Prepare and send necessary documentation to accepting agencies to ensure continuity of care.
- Coordinate transportation as instructed by the Case Manager.
- Demonstrate familiarity with local discharge planning resources.
- Document discharge planning activities in accordance with departmental policies.
- Maintain communication with case managers and social workers about the discharge planning status of all patients.
- Collaborate with the interdisciplinary care team to discuss patient discharge plans and needs.
- Work with case managers and social workers to confirm discharge dates and ensure transportation arrangements are in place.
- Ensure necessary authorizations for post-discharge services are obtained promptly.
- Keep case managers and social workers updated on the progress of assigned discharge planning tasks throughout the day.
- Adhere to safety protocols and report any unsafe conditions or practices to supervisors.
- Perform additional related duties as required.
- High School diploma or equivalent is required.
- A Bachelor’s degree in social work or a related field is preferred.
- Three years of experience in a supportive role within care management may substitute for an associate degree in social work or a related field.
- Experience in home health care or managed care is preferred.
- An equivalent combination of education and experience may be considered.
The ability to multitask effectively and thrive in a fast-paced work environment is essential.
