- Current LPN License: Possess a current and active Licensed Practical Nurse (LPN) license in the state where you will be practicing.
- Health Certificate: Hold a current health certificate, as required by the facility.
- PPD or Chest X-Ray: Have a current PPD or chest X-ray, as required by the facility.
- BLS Certification: Possess a current Basic Life Support (BLS) certification.
- Experience: One year of prior LPN experience is preferred, but not required.
