Position Overview:

The Housing Support Specialist collaborates with residents to secure safe alternative housing solutions or permanent accommodations swiftly, while connecting individuals to essential support services. The primary goal of the Housing Support Specialist is to resolve homelessness by promptly facilitating residents' transition to safe housing through the creation of tailored housing plans.

Core Responsibilities
  • Conduct an initial intake assessment within 72 hours of client admission, followed by a detailed evaluation after 7 days utilizing the VI-SPDAT in line with the CAS System in HMIS.
  • Formulate an Individual Housing Plan (IHP) within 10 days, focusing on:
    • Identifying client needs related to housing placement as outlined in the IHP (e.g., housing options, benefits, employment, health care, mental health, substance use, life skills, legal issues, etc.).
    • Recognizing strengths, challenges, and available resources.
    • Establishing clear target dates for housing placement, specifying responsibilities for each action step.
  • Initiate housing-focused case management services for all clients within 7 days.
    • After 7 days, the specialist will complete a Full VISPDAT to identify the most suitable housing options for the client.
  • Evaluate individual clients facing significant barriers at 30 days and request a case conference when necessary, adhering to the guidelines set forth by the CAS Policy. The specialist serves as the primary contact for housing placements.
    • Assist clients in overcoming barriers to housing, securing proper identification, obtaining income, settling debts, and applying for housing and subsidies.
  • Incorporate post-placement services in the IHP and follow up with clients after housing placement. Develop a housing stabilization strategy prior to exiting the shelter.
    • This will occur at intervals of 2 weeks, 4 weeks, 3 months, and 6 months post-placement.
  • Coordinate services with all providers and facilitate client reintegration into the community.
  • Utilize evidence-based strategies such as progressive engagement, motivational interviewing, and trauma-informed care to engage and motivate shelter clients.
  • Facilitate at least one life skills group session per month for clients (e.g., tenant rights, financial literacy).
  • Lead weekly community meetings with shelter residents, coordinating speakers, training sessions, and social events.
  • Maintain client records in compliance with funding sources and program standards as outlined in the Unified Shelter Policy Manual, including participation agreements, release forms, IHPs, and up-to-date progress notes.
  • Ensure confidentiality of client information at all times.
  • Collaborate as part of a team, regularly sharing information, strategies, support, and ideas to address the long-term needs of homeless individuals, including access to permanent housing.
  • Attend scheduled team meetings and other required gatherings.
  • Adhere to the agency's Code of Ethics.
  • Enhance skills through available training opportunities.
  • Perform other duties and projects as assigned.

  • Experience working with individuals facing serious mental health challenges and/or substance use disorders.
  • Proficient in Microsoft Word, Excel, and similar software.
  • Knowledgeable in Homeless Management Information System databases.
  • Capable of maintaining accurate records while ensuring confidentiality.
  • Demonstrates a commitment to diversity.
  • Exhibits sound judgment.
  • Reliable, dependable, and able to manage multiple tasks and deadlines.
  • Must possess a valid driver's license and personal liability insurance.
Experience and Educational Background
  • Bachelor's degree in Social Work or a related field is required.
  • Experience working with individuals facing substance use and mental health challenges, particularly those who are chronically homeless.
  • Familiarity with Motivational Interviewing and progressive methodologies.
  • Experience in multicultural environments.
  • CPR/First Aid Certification must be obtained and maintained upon employment.
Physical Demands:
  • Work is conducted in a shelter environment.
  • Must be able to lift at least 30 pounds.
  • Work may be subject to frequent interruptions.
  • Flexible hours may be required, including weekends, evenings, and holidays.
  • Work in various weather conditions may be necessary.
  • Proficiency in Word, Excel, and Homeless Management Information System databases is essential.
  • Candidate must have a valid driver's license and a clean driving record.
  • Must pass a negative Tuberculosis screening.
