Job Title: Senior Housing Outreach Specialist

We are seeking a highly motivated and compassionate Senior Housing Outreach Specialist to join our team at Goddard Riverside Community Center. As a key member of our outreach team, you will work closely with individuals experiencing homelessness, providing case management services and connecting them with permanent housing solutions.

About the Role:
  • Provide outreach services to homeless individuals, engaging them in harm reduction approaches and connecting them with supportive services.
  • Conduct intakes, assessments, and service planning to identify client needs and develop effective case management plans.
  • Collaborate with community partners, healthcare agencies, and other organizations to ensure seamless service delivery and achieve client placement goals.
  • Maintain accurate and up-to-date client records, utilizing handheld and desktop computers to track progress and outcomes.
  • Participate in outreach trips, drive outreach vans, and contribute to the annual HOPE count and quarterly counts of homeless individuals.
Requirements:
  • Bachelor's degree preferred, with prior experience working with homeless and/or mentally ill populations.
  • Valid driver's license and a driving record that meets the agency's auto insurance guidelines.
  • Flexibility in work hours, with availability to work evening, overnight, and weekend shifts as required.
