Job Summary

We are seeking a highly motivated and compassionate Outreach Independent Living Specialist to join our team at BRC. As a key member of our Transit Homeless Outreach program, you will provide comprehensive case management services to individuals experiencing homelessness, helping them to access essential services and achieve stable housing.

Key Responsibilities
  • Conduct outreach and engagement with homeless individuals in the transit system, building trust and rapport to facilitate access to services
  • Assess client needs and develop individualized housing plans to address barriers to housing
  • Provide case management services, including referrals to substance abuse programs, mental health services, and entitlements
  • Liaise with clinical staff and other stakeholders to ensure seamless service delivery
  • Document client progress and outcomes in a timely and accurate manner
Requirements
  • Bachelor's degree in a related field or significant experience in outreach or case management
  • Strong interest in working with homeless and/or mentally ill populations
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced, high-stress environment
  • Valid state driver's license and CPR training certification
