Job Summary

We are seeking a highly motivated and compassionate Senior Housing Outreach Specialist to join our team at Goddard Riverside Community Center. As a key member of our outreach team, you will work closely with individuals experiencing homelessness, providing case management services and connecting them with permanent housing solutions.

Key Responsibilities
  • Provide intake, assessment, and service planning for homeless adults
  • Develop and implement individualized case plans to promote housing stability and well-being
  • Collaborate with community partners and healthcare agencies to ensure comprehensive support services
  • Conduct outreach and engagement activities to identify and connect with homeless individuals
  • Maintain accurate and up-to-date client records and data entry
Requirements
  • Bachelor's degree in a related field (social work, psychology, etc.)
  • Experience working with homeless populations and/or mental health services
  • Valid driver's license and a clean driving record
  • Ability to work flexible hours, including evenings and weekends
