POSITION SUMMARY:

The Independent Living Outreach Specialist is responsible for delivering extensive case management services to individuals experiencing chronic homelessness. This role encompasses all facets of the assigned client caseload, which includes regular meetings with clients both in various settings and within the office environment. It is essential to maintain comprehensive and current clinical files and reports.

KEY RESPONSIBILITIES:

  • Conduct thorough assessments to identify housing barriers faced by clients.
  • Assist clients in overcoming these barriers and securing necessary entitlements.
  • Collaborate with clients to develop and progress on personalized housing plans.
  • Establish a supportive network of services tailored to client needs.
  • Prepare clients for housing interviews and coordinate housing appointments.
  • Participate in case conferences as required.
  • Act as a liaison with clinical staff from other programs regarding shared clients.

WORK SCHEDULE:

This is a full-time position requiring 37.5 hours per week, with a schedule from Tuesday to Saturday, 11 AM to 7:30 PM.

QUALIFICATIONS:

A Bachelor's degree in a related field is preferred. Significant experience in outreach or case management is strongly desired. A demonstrated commitment to providing direct services to homeless individuals, including those with mental health and substance abuse challenges, is essential. Candidates should possess the ability to assess and identify signs of mental illness and substance abuse, along with strong written and verbal communication skills. Proficiency in computer usage is required. The ability to thrive in a high-pressure environment while maintaining effective communication with the clinical team is crucial. Bilingual candidates are preferred, particularly in languages such as Spanish, Polish, Russian, Chinese, or Korean. A valid state driver's license is mandatory, and CPR training certification or a willingness to obtain it is required.

PROGRAM OVERVIEW:

The Transit Homeless Outreach program, funded by the Department of Homeless Services and the Metropolitan Transportation Authority, operates around the clock throughout the transit system. This initiative, which has been in operation since 2005 and expanded significantly in 2014, aims to provide humane outreach and case management services to assist homeless individuals in transitioning away from MTA properties. Our goal is to engage clients and connect them with housing and essential services.
