Goddard Riverside is seeking a highly motivated and compassionate Senior Housing Outreach Specialist to join our team. As a key member of our outreach team, you will work closely with individuals experiencing homelessness, providing case management services and connecting them with permanent housing solutions.
About the Role:- Provide outreach services to individuals living on the streets, utilizing a harm reduction approach to engage and support them.
- Conduct intakes, assessments, and service planning to identify clients' needs and develop personalized plans for housing placement.
- Collaborate with community partners, healthcare agencies, and other organizations to ensure a comprehensive approach to addressing homelessness.
- Maintain accurate and up-to-date client records, entering data into our computer database using handheld and desktop computers.
- Participate in outreach trips, identify and communicate with homeless clients, and drive the outreach van as needed.
- Attend staff meetings, case conferences, training programs, and supervision meetings to stay informed and contribute to program evaluation and improvement.
- Bachelor's Degree preferred.
- Prior experience working with homeless and/or mentally ill populations is a plus.
- Valid driver's license and a driving record that meets our auto insurance guidelines.
- Flexibility in work hours, including evening, overnight, and weekend shifts as required.
- A competitive salary of $41,987.40.
