Clinical Care Specialist

The Clinical Care Specialist is tasked with delivering direct patient services within rehabilitation and skilled nursing environments. This role is integral to the Post-Acute Care Team (PACT) assigned to specific Skilled Nursing Facilities (SNFs), focusing on enhancing patient outcomes while minimizing overall care expenses.
Key Responsibilities:
- Lead and coordinate care with hospitalists, primary care physicians, and SNF staff to ensure seamless transitions for patients.
- Collaborate with SNF personnel to create individualized post-SNF care plans, manage medication reconciliation, and facilitate communication with care managers.
- Work closely with the PACT medical director to ensure compliance with medical oversight and standards.
- Demonstrate adaptability and problem-solving capabilities while functioning independently and as part of a collaborative team.
Scope of Work:
The Clinical Care Specialist plays a vital role in addressing the medical needs of patients across multiple skilled nursing facilities located in close proximity. Establishing and nurturing strong professional relationships with facility administrators, nursing directors, and therapy staff is essential.
Patient Care Duties:
- Conduct patient assessments and provide direct care in rehabilitation and skilled nursing settings.
- Document patient interactions accurately using electronic health record systems.
- Be available for consultation with SNF staff and respond to patient inquiries during designated hours.
- Participate in an on-call rotation that may include evenings and weekends.
- Review and modify admission and discharge orders as necessary.
- Engage in discussions regarding palliative care with patients and their families.
Discharge Planning:
- Develop comprehensive discharge plans in collaboration with nursing staff, therapy providers, and social services.
- Identify and address barriers to discharge, ensuring a smooth transition for patients.
- Create detailed discharge summaries, including medication reconciliation and follow-up care instructions.
Qualifications:
- Master's degree in a relevant field from an accredited institution.
- Current licensure as a Physician Assistant (PA) or Advanced Practice Registered Nurse (APRN) in the state of practice.
- Eligibility for state and federal controlled substance licenses.
- Basic Life Support (BLS) certification and other relevant certifications.
Preferred Qualifications:
- Advanced degree in Geriatrics or related field.
- Three or more years of clinical experience in adult, family, or geriatric care.
- Experience in skilled nursing facilities or similar environments.
Physical Requirements:
Ongoing visual and auditory engagement with patients and care providers is essential. The role requires manual dexterity for operating medical equipment and the ability to lift and transport patients as needed. Frequent movement within busy environments is expected.
