Scope
The Clinical Care Specialist plays a vital role in addressing the healthcare needs of patients in skilled nursing facilities.
This position requires the establishment of robust collaborative relationships with facility administrators, nursing directors, and clinical staff across designated facilities.
The Specialist serves as the primary resource for medical inquiries and concerns for patients during their transitional stays.
Responsibilities
The Clinical Care Specialist will:
- Provide direct patient care within short-term rehabilitation and skilled nursing environments.
- Document patient interactions accurately using electronic health record systems.
- Ensure comprehensive care plans are maintained within the facilities' medical documentation systems.
- Be accessible via phone for facility staff during business hours.
- Respond to patient and family inquiries during designated hours.
- Participate in an on-call rotation that may include evenings and weekends.
- Review and adjust admission and discharge orders as necessary.
- Conduct in-person assessments for all patients post-admission.
- Facilitate discussions regarding palliative care with patients and their families.
- Verify and implement Health Care Proxy when appropriate.
- Consult with supervising physicians and facility medical directors as needed.
- Adhere to all regulatory requirements and maintain the confidentiality of protected health information (PHI).
- Meet expected patient load productivity and complete documentation within established timeframes.
- Identify and address gaps in clinical competencies among nursing staff, collaborating with leadership to create improvement plans.
Routine Visits
During routine visits, the Specialist will:
- Initiate and review treatment orders, including medications.
- Order necessary diagnostic tests and therapeutic interventions.
- Evaluate laboratory and radiology results.
- Conduct daily rounds to assess each assigned patient's medical stability.
- Assess any acute changes in mental status using appropriate measures.
- Coordinate with specialists and review their recommendations.
- Regularly communicate and assess rehabilitation progress.
- Engage with patients, families, and facility staff regarding illness management.
- Participate in family meetings to discuss care plans.
- Notify primary care physicians of significant medical changes.
- Take part in facility utilization meetings, including readmission reviews.
- Assess patients with acute or worsening conditions to determine appropriate treatment settings.
Discharge Activities
The Clinical Care Specialist will develop tailored discharge plans that incorporate input from nursing staff, therapy providers, and social services. This includes:
- Identifying and addressing barriers to discharge.
- Creating detailed discharge summaries, including medication reconciliation.
- Informing patients about the discharge process and ensuring implementation of the discharge plan.
Minimum Qualifications
To qualify for this role, candidates must possess:
- A PA or APRN degree from an accredited institution.
- Current licensure as a Physician Assistant or Advanced Practice Registered Nurse.
- Eligibility for State Controlled Substance License II through V.
- Eligibility for Federal DEA Certificate II through V.
- A valid driver's license.
- Basic Life Support Certification (BLS) for healthcare providers.
- Other required certifications (e.g., ACLS, PALS).
- Reliable transportation and an acceptable driving record.
Preferred Qualifications
Preferred candidates will have:
- A Master's degree or higher in Geriatrics.
- Certification from a recognized certifying organization.
- Three or more years of clinical nursing experience in relevant practices.
- Experience in skilled nursing facilities or nursing homes.
