Job Title: Private Duty Nurse LVN

Aveanna Healthcare is seeking a skilled and compassionate Private Duty Nurse LVN to join our team. As a Private Duty Nurse, you will provide one-on-one care to patients in their homes, assisting with daily living activities and providing medical support as needed.

Responsibilities:
  • Implement and update patient care plans in collaboration with physicians and other healthcare professionals.
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring.
  • Assess patient needs and develop individualized care plans to meet those needs.
  • Document patient care and progress in a timely and accurate manner.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements:
  • Current, unrestricted LPN/LVN license in the state of practice.
  • Current CPR certification.
  • One year of experience as a Private Duty Nurse or in a similar role.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
