Job Title: Private Duty Nurse LVN

We are seeking a skilled and compassionate Private Duty Nurse LVN to join our team at Aveanna Healthcare. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes, assisting with daily living activities and providing medical support as needed.

Responsibilities:
  • Implement and manage patient care plans in collaboration with physicians and other healthcare professionals
  • Conduct thorough patient assessments and develop individualized care plans to meet their unique needs
  • Administer medications and treatments as prescribed by physicians
  • Provide education and support to patients and their families on disease management and self-care techniques
  • Document patient progress and communicate with healthcare teams to ensure seamless care coordination
Requirements:
  • Current, unencumbered LPN/LVN license in the state of practice
  • Current CPR certification
  • One year of experience in pediatrics or home health care
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
