Job Summary:

Aveanna Healthcare is seeking a skilled and compassionate Private Duty Nurse LVN to provide high-quality patient care in the comfort of their own homes. As a Private Duty Nurse LVN, you will work closely with our patients and their families to develop and implement personalized care plans that meet their unique needs and goals.

Responsibilities:

  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Conduct thorough assessments of patients' physical and emotional needs, and document findings in a timely and accurate manner.
  • Administer medications and treatments as prescribed by physicians, and monitor patients' responses to care.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.

Requirements:

  • Current LVN licensure in the state of practice.
  • At least 6 months of experience as a LVN in a clinical care setting.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to lift 50 pounds and move equipment as needed.
