ROLE SUMMARY:

The Patient Problem-Solving Specialist plays a crucial role in the coordination efforts within the SPA 2 Coordinated Entry System (CES). This position acts as a vital link between healthcare facilities and homeless service organizations, fostering collaboration and facilitating smooth transitions of care for individuals experiencing homelessness. The specialist will assess high utilizers and ensure they are connected to the appropriate CES Access Site or Outreach Team. Additionally, this role involves working closely with internal departments and community partners to facilitate reconnections when necessary.

KEY RESPONSIBILITIES:

  • Develop and maintain strong collaborative relationships with hospital partners.
  • Provide training and education to hospital staff regarding CES resources and other relevant support for individuals facing homelessness.
  • Act as a liaison between healthcare providers and community service organizations.
  • Engage in hospital case conferences and collaborate on problem-solving initiatives for individuals experiencing homelessness.
  • Deliver problem-solving interventions to assist individuals in need.
  • Offer on-call technical support to hospital staff as required.
  • Track and assist participants in connecting with homeless services and re-engaging with providers as needed.
  • Assess high acuity frequent hospital utilizers and connect them with CES staff as appropriate.
  • Refer patients experiencing homelessness to CES Access Sites for evaluation and service connection.
  • Provide updates to homeless service providers regarding participants entering healthcare facilities.
  • Monitor metrics as assigned by the supervisor.
  • Document all participant interactions through case notes in compliance with clinical documentation and HIPAA standards, ensuring timely and accurate records for reporting in the HMIS database.
  • Encourage hospital involvement in relevant CES collaborative meetings.
  • Perform additional tasks and responsibilities as assigned by the supervisor.

QUALIFICATIONS:

  • Understanding of homeless populations, systems, and programs, particularly the Coordinated Entry System (CES).
  • Knowledge of the interrelationship between hospital systems and CES.
  • Familiarity with community resources available to individuals experiencing homelessness and how to access them.
  • Ability to work independently in various settings and as part of a team.
  • Strong relationship-building skills with stakeholders.
  • Effective presentation skills for diverse audiences.
  • Creative and solution-oriented problem-solving abilities.
  • Proficiency in Microsoft Office Suite and web-based software.

ADDITIONAL REQUIREMENTS:

  • Maintain confidentiality in accordance with HIPAA standards.
  • Obtain and maintain CPR/First Aid Certification.
  • Pass post-offer Tuberculosis (TB) clearances.
  • Regular travel is required for this position.
  • Possess a valid California Driver's License and maintain auto insurance.
  • Utilize a personal vehicle for travel between various locations.
  • Ability to work a 9/80 schedule.

PREFERRED EXPERIENCE:

  • Minimum of two years in a system coordination role.
  • Experience in a hospital or healthcare advocacy setting is preferred.
