Role Summary:

In the capacity of a Referral Coordinator, you will play a crucial role in the management and organization of the health center's referral processes. Working closely with the Program Manager, you will act as a vital connection between the clinic, medical facilities, and social service organizations, ensuring that patients receive coordinated and effective healthcare services.

Primary Duties:

  • Oversee the prompt and precise handling of all referrals, including the distribution of necessary documentation.
  • Coordinate appointment scheduling and verify that patients access the required medical care.
  • Enhance communication among healthcare providers, patients, specialists, and insurance plans.
  • Conduct follow-ups with patients, including reminders for upcoming appointments.
  • Uphold patient comfort and confidentiality at all times.
  • Provide clerical assistance, guidance on financial aid, and information on available resources to patients.
  • Address inquiries and concerns from patients regarding their referrals.
  • Stay updated on healthcare regulations and compliance standards.
  • Perform additional administrative tasks as needed.

Qualifications:

  • High school diploma or equivalent; a degree in healthcare administration is preferred.
  • Experience in a healthcare environment, especially in referral coordination, is advantageous.
  • Familiarity with EMR systems and relevant software applications.
  • Strong organizational skills and effective time management abilities.
  • Exceptional interpersonal and communication skills.
  • Meticulous attention to detail and accuracy in data management.
  • Understanding of medical terminology, insurance protocols, and referral systems.
  • Commitment to maintaining patient confidentiality and adhering to HIPAA regulations.
  • Ability to adapt and work collaboratively within a team setting.
  • Proficiency in MS Word and Excel applications.
  • Experience engaging with diverse populations; bilingual proficiency in English and Spanish is preferred.
