We are seeking a compassionate and service-focused RN or LPN for the role of Clinical Liaison in our hospice division.

The Clinical Liaison will serve as a crucial link between hospitals, healthcare facilities, physicians, and other referral sources, focusing on establishing strong relationships to foster effective communication and problem-solving. This role is essential in promoting our hospice services as a leading provider of high-quality care. The Clinical Liaison will conduct assessments, provide supportive care, and educate potential patients and their families about hospice options. Additionally, this position involves determining the appropriateness of hospice care for patients and facilitating their admission into the program.

This position requires on-site presence. The selected candidate will collaborate with our sales teams and will be expected to participate in meetings, events, and visits to various referral sources regularly.

Key Responsibilities:

  • Support Account Executives and Clinical Leadership in coordinating clinical care for referrals, patients, families, and facilities.
  • Evaluate and guide patients and families during initial hospice discussions to accurately assess care needs and eligibility.
  • Act as a subject matter expert on hospice eligibility criteria.
  • Complete all admission and consent documentation efficiently and accurately, demonstrating knowledge of the admission process.
  • Ensure that all necessary information and documentation are provided to patients and families, addressing any questions they may have.
  • Clearly explain hospice services and benefits to patients, families, and referral sources, including hospitals and physicians.
  • Assess and manage medication needs, overseeing orders for durable medical equipment, supplies, and oxygen as required.
  • Communicate and assist in resolving any issues that arise.
  • Provide discharge planning assistance from hospitals or facilities as necessary.
  • Collaborate with hospital and facility staff to determine discharge orders and facilitate a smooth transition to hospice care.
  • Relay important patient and family information to the interdisciplinary team to ensure a seamless transition.
  • Develop and maintain professional relationships with referral sources, attending meetings and community events to promote hospice services.
  • Document all patient interactions, referrals, and outcomes in the hospice's electronic medical record (EMR) system for accurate tracking and follow-up.
  • Train and support new staff in the hospice liaison role, including best practices for patient communication and managing referral sources.
  • Participate in interdisciplinary team meetings as required.
  • Advocate for patients in all aspects of the role, representing Envision in a professional manner.
  • Perform other duties and responsibilities as assigned.

Qualifications:

  • Preferred BA or BS degree; must possess an associate's degree in nursing or be a graduate of an accredited LPN program.
  • Current RN or LPN license in the state of Colorado is required.
  • 1-2 years of experience in hospice, home health, or palliative care is preferred.
  • Strong communication skills, both verbal and written, including group presentation abilities.
  • Must be computer literate and possess a reliable vehicle, valid driver's license, current car insurance, and a good driving record.
  • Experience in building and influencing client relationships, along with knowledge of Medicare managed care and insurance reimbursement, is advantageous.

Budgeted compensation for LPNs ranges from $31 to $36 per hour, while RNs are salaried between $74,000 and $83,000 annually.

This is a newly established role, and we are open to discussing flexible compensation options.
