Position Summary:
An immediate opportunity exists for a Clinical Liaison Registered Nurse (RN license required) to deliver patient care at various healthcare facilities in the Colorado Springs area.

Location: Colorado Springs, CO
Status: Full-time
Schedule: Monday-Friday, 9 AM - 6 PM
Compensation: $82,596 - $93,080 annually
Mileage Reimbursement: 62.5 cents per mile, one of the highest rates in the industry.

Key Responsibilities:
The Clinical Liaison will serve as a vital link between assigned hospitals, facilities, and physicians, focusing on:

  • Building effective relationships to enhance communication and problem-solving.
  • Providing assessments and education to prospective patients and families regarding hospice services.
  • Determining patient eligibility for hospice care and facilitating admissions.
  • Conducting thorough assessments to identify the physical and psychosocial needs of patients.
  • Completing admission paperwork and initiating clinical records accurately.
  • Coordinating medication needs and durable medical equipment orders.
  • Collaborating with healthcare staff to ensure smooth transitions into hospice care.

Qualifications:
We are looking for candidates who meet the following criteria:

  • Education: Graduate of an accredited nursing program; BSN preferred.
  • Experience: 1-2 years in hospice, palliative care, or related fields.
  • Licensure: Current Colorado RN license, valid driver's license, and CPR certification.

Physical Requirements:
Ability to lift a minimum of 30 lbs.
