Location: Colorado Springs, CO
Status: Full-time
Schedule: Monday-Friday, 9-6
Compensation: $82,596 - $93,080 annually
Mileage Reimbursement: Competitive rate of sixty-two and a half cents per mile.

Key Responsibilities:
The RN Clinical Liaison will serve as a vital link between assigned hospitals, facilities, and physicians, focusing on building strong relationships to enhance communication and problem-solving. Responsibilities include:
- Conducting assessments and providing supportive care and education to prospective patients and families regarding hospice services.
- Determining patient eligibility for hospice services and facilitating admissions.
- Gathering and reviewing patient information to assess physical and psychosocial needs.
- Completing admission paperwork and initiating clinical records through the EMR process.
- Managing immediate medication needs and coordinating durable medical equipment orders.
- Collaborating with hospital staff to ensure smooth discharge planning.
- Communicating essential patient information to the interdisciplinary team for coordinated care.
- Identifying patients eligible for General Inpatient services through hospital rounds.
Qualifications:
We are looking for candidates who meet the following criteria:
- Education: Graduate of an accredited nursing program; BSN preferred.
- Experience: 1-2 years in hospice, palliative care, medical/surgical, public health, oncology, acute care, or home health.
- Licensure: Current Colorado RN license, valid driver's license, auto insurance, and CPR certification.
Physical Requirements:
Ability to lift a minimum of 30 lbs.
