Position Overview:
Pikes Peak Hospice & Palliative Care is seeking a dedicated RN Clinical Liaison to join our team. This role requires a valid RN license and involves providing patient care at various healthcare facilities within the region.

Location: Colorado Springs, CO
Status: Full-time
Schedule: Monday-Friday, 9 AM - 6 PM
Compensation: $82,596 - $93,080 annually
Mileage Reimbursement: Competitive rate of sixty-two and a half cents per mile.

Key Responsibilities:
The RN Clinical Liaison will serve as a vital link between assigned hospitals, facilities, and physicians, focusing on:

  • Building strong relationships to enhance communication and problem-solving.
  • Providing assessments and education to prospective patients and families regarding hospice services.
  • Determining the appropriateness of hospice care and facilitating patient admissions.
  • Conducting initial interviews to assess the physical and psychosocial needs of patients.
  • Completing admission paperwork and initiating clinical records through electronic medical records (EMR).
  • Coordinating medication needs and durable medical equipment orders.
  • Collaborating with healthcare staff to ensure smooth discharge planning.
  • Educating and identifying patients eligible for General Inpatient services.

Qualifications:
We are looking for candidates who meet the following criteria:

  • Education: Graduate of an accredited nursing program; BSN preferred.
  • Experience: 1-2 years in hospice, palliative care, or related fields.
  • Licensure: Current Colorado RN license, valid driver's license, and CPR certification.

Physical Requirements:
Ability to lift a minimum of 30 lbs.
