Family Partner Job Description

Coordinated Care Services, Inc. (CCSI) is seeking a dedicated Family Partner to join our team. As a Family Partner, you will play a vital role in providing essential support services to families navigating multiple child-serving systems.

Key Responsibilities:

  • Assess risk factors and determine the most appropriate level of support for families
  • Meet with parents at mutually agreed-upon times and locations to identify strengths, needs, and potential services
  • Support parents in engaging identified services, including modeling, discussions, coaching, and skill-building
  • Consult with other team members regarding available service options and eligibility criteria
  • Maintain connections with parents as long as needed and mutually agreed upon
  • Attend regular supervisory meetings and participate in educational opportunities
  • Participate in meetings and community events, as needed

Requirements:

  • High school diploma/GED or equivalent education and work experience
  • Demonstrate 'lived experience' as a parent or primary caregiver who has navigated multiple child-serving systems
  • Previous experience working with youth and their families is highly desirable
  • Bi-lingual (Spanish/English) is preferred
  • Satisfactory completion of all required background screenings
  • Valid NYS Driver's License and/or access to reliable transportation
