Position Overview:
As a Hospice Licensed Practical Nurse (LPN), you will play a crucial role in delivering compassionate care to patients in their homes. Your responsibilities will include ensuring a seamless continuum of care and facilitating discharge planning. You will collaborate with an interdisciplinary team to provide direct patient support across various levels of care.

Key Responsibilities:
- Collect and document both objective and subjective patient information in line with agency protocols.
- Communicate relevant data to the supervising registered nurse and other healthcare professionals to initiate and adjust care plans as necessary.
- Engage with potential clients to explain the benefits and suitability of hospice services.
- Ensure that patients and their families are well-informed about hospice or homecare options.
- For hospice cases, manage incoming referrals effectively.
- Assist in patient assessments by gathering data for registered nurse analysis.
- Monitor patients for any unusual conditions and report findings to the appropriate staff.
- Identify safety risks and implement necessary precautions within your scope of practice.
- Administer medications as permitted and observe patients for symptom relief and any adverse reactions.
- Provide direct patient care, including dressing changes and laboratory tests, as part of the established care plan.
- Offer ongoing education and support to patients and their families, considering cultural and individual challenges.
- Maintain thorough and up-to-date documentation regarding patient conditions and the effectiveness of care interventions.
- Facilitate a smooth transition to residential settings by coordinating services during discharge.
- Adapt daily priorities based on patient needs, family requests, and available resources.
- Perform additional duties as assigned.


Qualifications:
- Graduate of an accredited practical or vocational nursing program.
- Current Ohio License as a Licensed Practical Nurse is required; M-IV certification is preferred.
- Basic Life Support (BLS) certification from the American Heart Association (AHA).
- Valid Ohio Driver's license and proof of automobile insurance are required.

Competencies:
- Familiarity with JCAHO and Medicare standards and regulations.
- Ability to work efficiently in a fast-paced environment while maintaining accuracy.
- Strong relationship-building skills with patients, physicians, and community organizations, ensuring excellent service delivery.
Experience:
- Minimum of one year of clinical experience as a professional nurse in an acute care setting. Previous experience in hospice or homecare, as well as infusion experience, is preferred.

Physical Requirements:
- Full range of motion, manual dexterity, and coordination are necessary.
- Ability to stand and walk for extended periods.
- Requires corrected hearing and vision within normal ranges.
- Some exposure to communicable diseases or bodily fluids may occur.
- Must be able to exert physical force as needed for patient care tasks.
- Must be able to drive to patient locations and navigate stairs as required.
