About the Role

We are seeking a compassionate and skilled Hospice Aide/CNA to join our team at Adoration Home Health and Hospice. As a Hospice Aide/CNA, you will play a vital role in providing personalized care and support to patients and their families in the comfort of their own homes.

Responsibilities
  • Provide personal care services, including bathing, dressing, and grooming, as needed
  • Assist with daily living activities, such as feeding, toileting, and transferring
  • Monitor and report changes in patient condition to the Case Manager
  • Assist with light housekeeping and laundry duties
  • Provide emotional support and companionship to patients and their families
  • Collaborate with the healthcare team to develop and implement individualized care plans
Requirements
  • Meets the training requirements of the State
  • Completed a minimum of seventy-five hours of classroom and supervised practical training
  • Minimum of six months experience in providing personal care
  • At least eighteen years of age
  • Ability to read and follow written instructions and document care given
  • Understands hospice philosophy and is comfortable providing specialized care to the terminally ill
