Gentiva Hospice is seeking a compassionate and dedicated Hospice Care Specialist to join our team. As a Hospice Care Specialist, you will play a vital role in providing emotional and physical support to patients and their families during a challenging time.

Key Responsibilities
  • Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming.
  • Assist patients with ambulation and mobility, using walkers or wheelchairs as needed.
  • Prepare and serve light meals and snacks, and assist with feeding patients when needed.
  • Maintain a safe and healthy environment, including light housekeeping tasks such as emptying trash and changing linens.
  • Document patient care and progress, and report any changes or concerns to the Registered Nurse and interdisciplinary team.
Requirements
  • Certified Nursing Assistant (CNA) license.
  • 6 months of patient care experience in hospice, home health, ICU, geriatric, or oncology.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
