We strive to deliver the most comprehensive solutions for end-of-life care through exceptional hospice operations.

Key Responsibilities In your role as a Hospice Social Worker, you will:

  • Perform thorough psychosocial evaluations of patients and their families.
  • Offer individual and family counseling to address issues related to grief, loss, and end-of-life choices.
  • Assist in advance care planning and provide decision-making support.
  • Link patients and families to community resources and support services.
  • Deliver bereavement support and follow-up care for families after the passing of a loved one.

As a Bereavement Coordinator, your responsibilities will include:

  • Assessing the needs of bereaved family members and caregivers.
  • Providing individual and group counseling throughout the grieving journey.
  • Facilitating grief support groups and memorial events.
