Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Hospice Care Specialist. As a key member of our patient care team, you will be responsible for providing high-quality care and support to our patients, while also collaborating with our interdisciplinary team to ensure seamless delivery of services.

Key Responsibilities
  • Provide comprehensive nursing care to patients in their homes, assisted living facilities, and hospitals
  • Conduct thorough assessments and develop individualized care plans to meet the unique needs of each patient
  • Collaborate with our interdisciplinary team, including social workers, chaplains, and volunteers, to ensure comprehensive care and support
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless delivery of services
  • Manage patient admissions, transfers, and discharges, ensuring timely and efficient care
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements
  • Current Registered Nurse licensure in good standing in the state(s) of practice
  • At least one year of experience as a Registered Nurse, preferably in hospice or palliative care
  • Excellent clinical and communication skills, with the ability to work effectively in a fast-paced environment
  • Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage multiple responsibilities
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and communication
