Sun River Health is seeking a Wellness Liaison to join our team in Jamaica Queens, NY. The Wellness Liaison will provide outreach and education services to coordinate intake and enrollment into Health Home and all Sun River Health service lines and programmatic services.
Key Responsibilities:- Participate in outreach and educational events
- Serve as liaison between clients, providers, and account management staff
- Provide outbound reminder calls to clients prior to their scheduled appointments
- Assist clients in securing appointments for SRH Health Center services
- Conduct Health Home pre-eligibility screenings and intakes for eligible clients
- Achieve intake monthly targets
- Aide with client outreach and education of SRH services to community
- Assist with health insurance application and transportation arrangements
- Responsible for accurate and timely entry of data into required databases
- Provide necessary documentation and information to Departmental Health Home Manager
- Participate in community events and health fairs
- Act as initial service ambassador and promote SRH services
- Support SRH's mission, values, and high standards of customer service
Requirements:
- Bachelor's degree preferred, High School Diploma/GED required with additional 4 years of relevant experience
- 2-4 years of work-related experience in customer service
