Wellness Liaison

Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia, and Westchester County. We are seeking a full-time Wellness Liaison to join our team in Jamaica Queens, NY.
Job SummaryThe Wellness Liaison is responsible for providing outreach and education services to coordinate intake and enrollment into Health Home and all Sun River Health service lines and programmatic services. The position requires strong interpersonal skills to make connections and build relationships with the target patient/client base.
Key Responsibilities- Participate in outreach and educational events
- Serve as liaison between clients, providers, and account management staff
- Provide outbound reminder calls to clients prior to their scheduled appointments
- Assist clients in securing appointments for SRH Health Center services
- Conduct Health Home pre-eligibility screenings and intakes for eligible clients
- Achieve intake monthly targets set on an annual basis and adjusted quarterly as needed
- Aide with client outreach, education of SRH services to the community with a focus on linkage to Health Center and Health Home
- Assist in arranging transportation when needed
- Assist with health insurance application when needed
- Responsible for the complete, accurate, and timely entry of data into required databases (eCW, TREAT, Salesforce-CRM)
- Provide necessary documentation and information to Departmental Health Home Manager in each county/borough
- Participate and attend Community Events such as health fairs; conduct on-street marketing of flyers and service information pamphlets
- Provide support of Health Center and Health Home by participating in Kiosk/Mobile events in specified locations on schedule to engage community and conduct Health Home Intakes for health fairs, special events, and locations
- Act as initial service ambassador on the promotion and education to referral sources, clients, and community members of all SRH services while providing the highest quality service
- Supports SRH's mission, values, strategic goals, and high standards of customer service
- Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health information
- Participates in team meetings
Minimum Education Requirement: Bachelor Preferred, High School Diploma/GED required with an additional four years of relevant experience in field of customer service in appropriate service industries
Minimum Work Related Experience: 2-4 years
Job Type: Full-time
Salary: $24.00 per hour
