Position Details:

This is a full-time role, operating Monday through Friday from 8:00 AM to 4:30 PM.


Role Responsibilities:

  • Actively participate as a vital member of the interdisciplinary team (IDT), ensuring all elements and tasks related to the Social Services Department are tracked and managed effectively.
  • Engage in daily IDT meetings, providing updates on tasks and responsibilities related to social service coordination for participants.
  • Oversee and organize the schedule for in-person assessments, including evaluations of changing conditions, service requests, and care conferences.
  • Prepare the Social Worker for assessments and care planning meetings.
  • Present care plans to participants and secure necessary signatures for documentation.
  • Administer necessary psychometric evaluations as required.
  • Foster strong relationships with participants and their families to ensure high-quality, person-centered care.
  • Coordinate family meetings as required.
  • Monitor and document individual interventions as outlined in care plans.
  • Notify staff of any crises occurring within the center or community.
  • Maintain accurate and timely case management records, documenting all interactions and services provided.
  • Serve as a liaison between participants and external agencies.
  • Assist in obtaining necessary documentation for financial assistance programs.
  • Participate in community conferences related to participant care.
  • Uphold confidentiality regarding participant information.
  • Attend regular meetings with supervisors and departmental gatherings.
  • Engage in all staff meetings, training sessions, and assigned projects.
  • Adhere to the center's policies and procedures.
  • Be adaptable in work hours as needed.
  • May require the use of a personal vehicle.

Qualifications:

  • High School Diploma or equivalent.
  • Current Basic Life Support (BLS) certification is mandatory.
  • Valid Driver's License with current automobile insurance.
  • Bilingual in English and Spanish is preferred.

Experience and Skills:

  • At least one year of experience working with elderly or frail populations is essential.
  • A minimum of one year in a behavioral health or social services environment is required.
  • Excellent written and verbal communication skills are necessary for effective interaction with all stakeholders.
  • Ability to build and maintain productive relationships with participants, colleagues, and external partners.
  • Proficiency in Microsoft Office applications and familiarity with Electronic Medical Record systems is advantageous.
  • Commitment to compliance with all relevant health and safety regulations.
  • A passion for working in the social services field, particularly with the geriatric demographic.

Health Requirements:

All employees must provide medical clearance for communicable diseases and maintain up-to-date immunizations, including proof of Covid vaccination. Employment offers are contingent upon successful completion of a pre-employment drug screening, background check, and physical assessment.
