Role Overview:
The primary aim of this position is to support the planning, development, organization, implementation, evaluation, and management of our facility's social service initiatives in compliance with current federal, state, and local regulations, as well as our established policies and procedures, ensuring that the emotional and social needs of residents are met on an individual basis.
Key Responsibilities:
- Assist the Social Service Director/Consultant in the planning, development, organization, implementation, evaluation, and management of the facility's social service initiatives.
- Help in formulating and executing policies and procedures for identifying the medically related social and emotional needs of residents.
- Engage in community planning related to the interests of the facility and the services and needs of residents and their families.
- Support the provision of medically related social services to ensure the highest possible physical, mental, and psychosocial well-being of each resident is achieved or maintained.
- Complete administrative tasks, such as filling out necessary forms and reports, and submitting them to the Director.
- Provide information to residents and families regarding Medicare/Medicaid and other financial assistance programs available.
- Offer consultation to staff members and community agencies to address the needs and challenges faced by residents through the development of social service programs.
- A minimum of a high school diploma or equivalent is required; an Associate or Bachelor's Degree is preferred but not mandatory. Previous healthcare experience is advantageous.
- Must be able to pass a criminal background check as well as a Colorado CAPS background check.
- COVID vaccination is required or a valid exemption must be provided.
- Experience in long-term care is necessary.
Compensation: $22 - $26 per hour
