Reports To: Supervising Care Manager (SCM)
Summary: Responsible for leading the DAS Case Management initiative and delivering case management services to elderly individuals and adults with disabilities.
Key Responsibilities:
- Collaborate with SCM to manage the Case Management Program and Short-Term Home Care for Older Adults initiative within DAS.
- Mentor, oversee, and assess the performance of the program's personnel.
- Deliver case management services to seniors and adults with disabilities, which encompass:
b. Executing a thorough evaluation of the client's functional and cognitive abilities during home visits;
c. Formulating a care plan based on the evaluation;
d. Executing the care plan by coordinating and arranging necessary services;
e. Monitoring and reviewing the plan to ensure it continues to address the client's needs;
f. Regularly reassessing the client's requirements; and
g. Concluding case management services when suitable.
- Maintain comprehensive client records and complete all mandatory data collection forms and reports.
- Engage in evaluation and quality assurance processes to assess the effectiveness of the Social Services programs as needed.
- Take charge of personal professional development and ongoing education to enhance case management capabilities; contribute to the Manager/Director in formulating the training strategy to fulfill both individual and team training requirements.
- Participate in case discussions, training sessions, and other necessary meetings.
- Build networks and conduct outreach to community organizations, other service providers, and families and caregivers; present at community gatherings and other relevant venues to inform the public about the agency's offerings.
- Execute additional responsibilities as assigned.
Qualifications:
- Bachelor's degree in health, social work, counseling, psychology, gerontology, or related fields with significant case management experience and some administrative/supervisory experience. A master's degree is highly preferred.
- Possess the knowledge, skills, and experience necessary to evaluate the client's service needs and to perform essential case management functions.
- Demonstrate sensitivity to the needs of elderly and disabled adults from diverse ethnic and minority backgrounds.
- Exhibit excellent communication and interpersonal abilities.
- Must be organized and capable of managing multiple tasks simultaneously.
- Ability to work independently as well as collaboratively within a team.
- Bilingual and bi-literate in English and Chinese.
- Must be able to travel using public transportation or personal vehicle for job-related duties. If driving, must possess and maintain a valid CA driver license, a satisfactory driving record, and auto insurance coverage as per Self-Help's policy.
