Job Overview

This is a full-time role, with a schedule from Monday to Friday, 8:00 AM to 4:30 PM.


Position Overview:

Under the guidance of the Behavioral Health Director, you will oversee and facilitate all functions within the Social Services Department.

Key Responsibilities:

  • Actively participate as a collaborative member of the interdisciplinary team (IDT), ensuring all tasks related to the Social Services Department and assigned MSW are tracked and managed.
  • Attend daily IDT meetings to provide updates on tasks and assignments concerning social service coordination for all participants.
  • Manage the MSW schedule for in-person evaluations, including assessments for changes in conditions, service requests, and care conferences.
  • Prepare the MSW for various assessments and meetings, ensuring all necessary documentation is ready.
  • Facilitate care planning meetings with the IDT and present care plans to participants for their approval.
  • Administer necessary psychometric assessments, such as PHQ-9 and GAD-7, as required.
  • Build and maintain strong relationships with participants and their families to ensure high-quality, person-centered care.
  • Coordinate family meetings as needed to discuss participant care.
  • Monitor interventions outlined in care plans and maintain accurate medical records.
  • Notify SW/BH staff of any crises occurring within the center or community.
  • Maintain thorough and timely documentation of all participant interactions and services provided.
  • Serve as a liaison between participants and external agencies, including the Department of Aging and Medicaid.
  • Assist in obtaining necessary financial documentation from participants.
  • Participate in community conferences related to participant care as assigned.
  • Ensure confidentiality of all participant information.
  • Attend regular meetings with supervisors and mandatory department gatherings.
  • Engage in all staff meetings, training sessions, and projects as required.
  • Comply with the center's policies and procedures.
  • Be adaptable to changes in work hours as needed.
  • Use personal transportation as required for job duties.

Qualifications:

  • High School Diploma or GED is required.
  • Current Basic Life Support (BLS) certification is mandatory.
  • A valid Driver's License and current automobile insurance are required.
  • Bilingual in English and Spanish is preferred.

Experience and Skills:

  • At least one year of experience working with elderly or frail populations is essential.
  • A minimum of one year of experience in a behavioral health or social services environment is required.
  • Excellent written and verbal communication skills are necessary for effective interaction with all stakeholders.
  • Ability to foster effective working relationships with participants, colleagues, and the public.
  • Proficiency in Microsoft Office applications and familiarity with Electronic Medical Record systems is preferred.
  • Adherence to all applicable health and safety regulations is required.
  • A genuine desire to work in the geriatric care field is essential.

Health Requirements:

Employees must provide medical clearance for communicable diseases and maintain up-to-date immunizations, including proof of Covid vaccination upon hire. Employment offers are contingent upon successful completion of a pre-employment drug screening, background check, and physical assessment.
