Position Overview:

The Enrollment Specialist plays a crucial role in the St. Paul's PACE program, which is dedicated to providing comprehensive care for seniors aged 55 and older who are managing chronic illnesses or disabilities. This position is responsible for facilitating the intake process and ensuring that potential participants receive the necessary information and referrals to appropriate community resources.

Key Responsibilities:
  • Gather initial information related to admissions and coordinate the intake process.
  • Promote the St. Paul's PACE Program to prospective participants and community organizations.
  • Provide information and referrals to community agencies as needed.
Qualifications:

Education: A Bachelor's Degree in business, health, or human services, or a high school diploma accompanied by relevant office assistant training.

Experience: A minimum of two years in a marketing or admissions role within a healthcare setting, or experience in information and referral services within the senior network.

At least one year of experience working with elderly or frail populations is required.

Skills:
  • Ability to effectively communicate the PACE philosophy to the community and potential participants.
  • Knowledge of the physical, psychosocial, and behavioral needs of the elderly.
  • Familiarity with community health and social service systems in the area.
  • Strong written and verbal communication skills.
  • Understanding of Medicaid and financial assessment processes.
  • Proficiency in word processing, data entry, and spreadsheet applications.
Physical Requirements:

Must maintain good mental and physical health, capable of performing assigned tasks, including lifting and handling patients. The role requires mobility, including standing, walking, bending, and reaching in various settings.
