Job Description

Prestige Staffing is seeking a detail-oriented and proactive Care Coordination Specialist to manage a high volume of incoming referrals and facilitate smooth patient onboarding.

Key Responsibilities:

  • Manage and process incoming referrals and patient intake, ensuring timely and accurate information capture.
  • Collaborate with care teams, including doctors, PAs, and clinical assistants, to streamline the intake process and address any backlog of referrals.
  • Utilize EMR systems (Allscripts/Veradigm) and their Intake System (Phreesia) for managing patient information and processing documentation.
  • Handle demographic data, schedule appointments, and coordinate with clinical assistants on administrative and clinical tasks.
  • Support the transition and onboarding of patients, ensuring a seamless integration into the care system.

Requirements:

  • High school diploma or equivalent; relevant experience in healthcare administration or a related field is preferred.
  • Experience as a Medical Assistant is helpful
  • Experience in intake coordination, preferably with knowledge of infusion and neurology or a strong foundation in primary care.
  • Proficiency with EMR systems, specifically Allscripts/Veradigm, and experience with Phreesia.
  • Strong organizational and multitasking skills, with an ability to manage a high volume of referrals efficiently.
  • Excellent communication skills and a collaborative approach to working with care teams.

Additional Skills:

  • Clinical knowledge or experience, particularly in areas relevant to intake processes, is a plus.
  • Ability to handle administrative tasks with accuracy and efficiency while maintaining a high level of patient care.
  • Flexibility and adaptability to work effectively in a transitioning environment.
