Job Summary

CarePartners is seeking a skilled Post-Acute Transition Specialist to join our team. This role will act as a coordinator between internal and external referral sources and CarePartners Post-Acute Service Lines.

Key Responsibilities
  • Establish relationships with hospitals, skilled nursing facilities, assisted living facilities, ambulatory and inpatient care managers, physicians, payors, and other community referral sources.
  • Provide education and promote collaboration to ensure appropriate patient placement.
  • Work closely with Post-Acute providers, SNF's, and physician practices in the clinically integrated network (CIN) to ensure a responsive and smooth flowing referral and admission process.
  • Collaborate with health system and community colleagues to insure quality driven, best practice across the continuum.
  • Maximize positive financial outcomes for designated patient populations by facilitating appropriate Post-Acute placement and participating in Goals of Care/Advanced Care Planning dialogue.
Requirements
  • Bachelor of Science in Nursing or related field.
  • Minimum of two years recent experience in a health-related field.
  • License in therapy discipline (if applicable).
  • National Nursing Certification in Case Management (ACM or CCM, or ANA approved certification) (if applicable).
