Overview:

  • The Patient Care Coordinator offers essential psychosocial assistance to individuals, families, and groups facing chronic, acute, or terminal health challenges. This includes guidance for family caregivers.
  • Provides patients with necessary information and counseling, along with referrals to additional services.
  • Reports directly to the Unit Supervisor.
Key Responsibilities:
  • Conducts admission, concurrent, and retrospective evaluations for all patients.
  • Ensures accurate status and level of care determinations upon patient admission.
  • Secures and documents pre-certifications/authorizations for all admissions, surgeries, and procedures.
  • Adheres to Medicare guidelines for utilization reviews.
  • Submits clinical information via telephonic and/or fax notifications.
  • Follows up with insurers for necessary authorizations.
  • Identifies high-risk patients and refers them to Social Work.
  • Delivers comprehensive clinical documentation and performs Initial Assessments to formulate care plans.
  • Ensures treatment plans are implemented, reassessed, and updated on a daily basis.
  • Facilitates effective communication among treatment team members.
  • Coordinates meetings with families and the treatment team.
  • Arranges post-acute care services.
  • Acts as an advocate for patients and their families.
  • Complies with Medicare guidelines regarding patient choice, admission/discharge notifications, and appeals processes.
  • Maintains accountability for guiding clinical patient progression through a defined care plan to achieve optimal outcomes.
  • Documents and reports quality, cost, and risk variances related to the patient's care plan.
Qualifications:
  • Graduate from an accredited nursing program.
  • Current license in good standing.
  • At least one year of recent experience as a Case Manager in a healthcare facility.
  • Current AHA BLS certification is mandatory.
