Position Overview

Scripps Health Administrative Services plays a vital role in supporting our extensive network of healthcare facilities, which serve a significant number of patients annually through a dedicated team of healthcare professionals.

This is a Full-Time position with a flexible schedule that includes weekend and holiday shifts. Typical working hours are from 7:30 AM to 6:15 PM.

Scripps Memorial Hospital La Jolla has a long-standing reputation as a leading medical institution in Southern California, being the first in the region to achieve Magnet Hospital designation.

As a Patient Care Coordinator, you will oversee the daily management and organization of care for patients with identified immediate post-acute care requirements. This includes collaboration with healthcare providers, navigators, and community resources throughout the care continuum. Additionally, you will:

· Address insurance-related issues that may affect patient care and develop a comprehensive transition plan from inpatient to post-acute care, ensuring a safe discharge process.

· Confirm that the patient and their family are in agreement with the transition plan and that it is effectively executed.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN) is highly preferred.
  • Experience in Case Management or recent acute care hospital settings.
  • Case Management certification is highly desirable.
  • Familiarity with EPIC systems is preferred.

Required Qualifications:

  • Registered Nurse with a minimum of 2 years of clinical experience.
  • 1-2 years of experience in inpatient acute hospital case management.

Required Certifications:

  • Current California RN License.
  • Current BLS certification for Healthcare Providers from the American Heart Association.

The following candidates are not eligible for hiring incentives:

  • Internal applicants.
  • Rehires who left Scripps less than three years ago.

To maintain eligibility for retention incentives, the following criteria must be adhered to:

  • Must retain original employment status and shift as specified in the offer letter.
  • Must remain in the original department or specialty.
  • Must maintain the original job title.
  • Transfers to other locations will be evaluated on a case-by-case basis and may result in forfeiting remaining incentive bonuses unless explicitly stated in the offer letter.
